In order to register for a class at St. Matthew's Weekday School, you must complete and turn in the following forms:

Commonwealth of Virginia School Entrance Health Form, all parts including the Health Information Form, Certification of Immunization, and Comprehensive Physical Examination Report.  If your child has an allergy, especially a food allergy, then please be sure to have your physician complete an Allergy Action Plan.

Additionally, you must bring in your child's original birth certificate so that it can be copied for his or her file. 

SMUMC WDS Scholarship Form, The St. Matthew’s United Methodist Church Weekday School Board and Director review all applications and, with the St. Matthew’s UMC Legacy Committee, determine scholarship recipients and amounts. The scholarship is available throughout  the school year and is available due to various circumstances and financial hardships.

SMUMC WDS Parent Handbook, The St. Matthew’ s Weekday School Handbook is updated annual for parents review. We require a signed acknowledgement of receipt at the beginning of every school year.

Allergy Action Plan, complete this form to specify any allergies that require epinephrine for any anaphylaxis reaction including Epi-Pen, Auvi Q, Benadryl or any other life-threatening emergency medication. At this time we are trained to administer epinephrine, Benadryl, nebulizers and inhalers. WE ARE A NUT FREE PRESCHOOL.
